Causes of events

The most frequent causes of notified significant events

are the regulation contamination limits on the package or

conveyance being exceeded. Steps have been takenby the

industry to reduce the number of these events.

The other causes of significant events recorded include:

documentary, package labelling and vehicle placarding

errors;

defective stowage or handling accidents, which can lead

to package damage;

theft or loss of radiopharmaceutical packages.

The breakdown of notified transport events according

to package content and mode of transport is illustrated

in graph 4.
